We need written permission to enrol you into these scheme, so please sign off the form below to confirm that you have been made aware of this and understand your obligations.

Enrollment is mandatory with some exceptions (very low earnings, under 22 etc). In some cases you are allowed to opt out, for example you may already be paying into a qualifying scheme.

Our recommendation is to enrol.
